From Azaoomachi, Kousamachi Kamimashikigun, Kumamoto Prefecture (AO)
Note: September 21, 1928. Findings of investigation of the accident which occurred in the No. 2 mine, Hanna, property of the Union Pacific Coal Company, and resulted in the death of G. Ikegami, Japanese. We found that G. Ikegami was employed in the 11th Entry in Room 4 as a miner. He was working close to the face of the room shoveling and throwing coal into the chute when a large piece of rock, 8 feet by 5 feet, by 13 inches fell from the roof and caught him squarely causing instant death. Note: Fatal: Miner [G. Ikegami] had walked up to working face (pillar) and was about to shovel coal in chute when he was caught and buried by a fall of coal and rock. Death was instantaneous. It was an unavoidable accident.

Note: G. Ikegami, a miner at Hanna, was struck by a fall of coal and killed instantly. This was one of the few unavoidable accidents of the month. Mr. Ikegami's death was the second at Hanna during the year and the fourth to occur on the system. (UPCCEM, November 1928)
Note: The funeral of G. Ikegami, Japanese, killed by a fall of rock in No. 2 Mine, was held at the Methodist Church on Sunday, September 23rd. The deceased was 50 years of age and had lived in Hanna a number of years. He leaves to mourn his loss a wife and daughter in Japan. Interment was made in the Hanna cemetery.
Note: Gitsuji Ikegami was killed in the Union Pacific Coal Company's No. 2 mine in Hanna. He was buried in Plot 303.1 in the Hanna cemetery. (BL)

Hanna Monument: At the top of Hanna No. 1 Hill stands a Memorial Monument dedicated to the men who lost their lives in the explosions of the Union Pacific Coal Company's No. 1 Mine in Hanna. The Monument also marks the area where 28 of the men - like John Ikonen - were killed, but not recovered and their bodies remain at the bottom of the mine.
Note: John Ikonen worked as a Rollerman in the No. 1 mine. On Saturday, March 28th, while the mine was idle, a fire crew consisting of 18 men including John entered the Number 1 mine to put out a fire burning in entry number 10. The mine exploded about three in the afternoon killing all the men in the fire crew. A rescue crew was organized and entered the mine to search for survivors. A second explosion occurred the same day about ten thirty in the evening killing another 41 miners. John's body was not found it is still at the bottom of the No. 1 mine. (BL)

Note: The sad accident by which William Isaacs and Martin Kain lost their lives a few days ago, cast a feeling of gloom over the entire community from which it has not yet rallied. Life in the coal mines after an accident of this kind seems precarious, and the grimy, hard-fisted fellows who go down every morning to dig fuel for the upper world certainly deserve the grateful thanks of us who have nothing to do but to sit in our houses and burn it. Both Isaacs and Kain were old members of the fraternity of Old Fellows and were buried by Carbon Lodge, which turned out in force accompanied by a large number of citizens.
